Care Coordination is an increasingly significant concept in the health care industry. It includes helping patients navigate the complex health care system, such that patients have a more optimal experience and improved health outcomes. In this course, students will engage in lectures, group activities, and case studies that will train them to better communicate with and recognize the needs of patients. When students complete this course, they should be able to help patients receive timely and appropriate care, guide patients to engage more fully in their own care, and ensure that communication takes place between all the providers involved in a patient’s health care experience.

1. Identify any barriers or potential barriers to a patient's health care experience
2. Assist patients, caregivers, and families with services, resources, counseling, and financial issues

Methods by which student learning will be assessed and evaluated; describe the types of methods to be employed; note whether certain methods are required for all sections:
This course will involve homework assignments, case studies, group exercises and discussions that require both oral and written communication about students' experiences with health care both as patients and as future health care workers. Students will also be required to perform role playing scenarios in the classroom.For homework assignments, students will read articles and answer questions based on the articles or do case studies. The instructors can rate students' ability to communicate effectively and practice patient-centered and coordinated care through these written, oral, and role playing assignments. Further, quizzes will be delivered for objective assessment. These methods will be required for all sections.
Grade distribution for the overall grade will be based on the following criteria:
In-class activities 50%
Homework Assignments 20%
